FSP vs Kenntnisprüfung: Which Exam Do You Need?

Two exams, two very different purposes. Here is how to know which one applies to you — and how to prepare for each.

One of the most common questions international doctors ask us is: "Do I need the FSP, the Kenntnisprüfung, or both?" The answer depends on your country of qualification, your Bundesland, and how your medical degree is assessed. Let us clear this up.

The FSP: Testing your medical German

The Fachsprachprüfung (FSP) is a language exam. It tests whether you can function as a doctor in German — take a patient history, discuss a case with a colleague, and write clinical documentation. Every international doctor needs to pass the FSP, regardless of where they qualified. There are no exceptions. Even if your German is perfect, you still sit the FSP.

The FSP is administered by the Ärztekammer of the federal state where you apply. The format is similar across states, but difficulty and pass rates vary. Berlin and Hamburg are considered among the stricter Kammern; some smaller states are perceived as more lenient, though the exam content is comparable.

The Kenntnisprüfung: Testing your medical knowledge

The Kenntnisprüfung is a knowledge exam. It tests whether your medical education is equivalent to the German standard. It covers internal medicine, surgery, emergency medicine, pharmacology and medical law — all conducted in German. You only need the Kenntnisprüfung if the Landesprüfungsamt determines that your degree is not fully equivalent to a German medical degree.

In practice, most non-EU doctors are directed to the Kenntnisprüfung. EU/EEA graduates are usually exempt because their degrees are automatically recognised under EU directives. But there are exceptions on both sides — some non-EU degrees from well-known universities are recognised without the Kenntnisprüfung, and some EU graduates from newer programmes may face additional requirements.

How to find out which exam you need

The only authoritative answer comes from the Landesprüfungsamt of the state where you apply. When you submit your Approbation application with your translated documents, they assess your qualification and issue a Bescheid (decision) telling you exactly which exam(s) you need. However, based on experience, we can usually predict the outcome based on your country and university — and save you months of uncertainty.

Can you need both?

Yes. If your degree is not fully recognised, you need the Kenntnisprüfung (knowledge) AND the FSP (language). The order does not matter in most states, but we generally recommend passing the FSP first — it is faster to prepare for, and passing it gives you momentum and confidence.

How preparation differs

FSP preparation is focused on clinical communication — vocabulary, patient scenarios, structured case presentations and Arztbrief writing. Six weeks of intensive preparation is usually sufficient for a candidate with solid B2 German. Kenntnisprüfung preparation is focused on medical knowledge revision — across multiple specialties, in German. Eight weeks is typical, but candidates with gaps in specific areas may need longer.
